Advance Care Planning
Peace of mind for your journey ahead
Advance care planning is the process of thinking about, discussing and documenting your wishes about what medical care and treatment you would and would not want if you were to become seriously ill or involved in a serious accident, and who you would want to make medical decisions on your behalf if you were unable to make them yourself.
My clients are just like you
Contact me if you are interested in an advance care planning workshop for your group:
community members
community professionals
health care organizations
families
My clients are individuals, couples and multi-generational families, of all ages and states of health. Together we discuss the process of advance care planning and the related forms and documents.
If you are a client living with illness, we discuss where you are on your journey. I support you in approaching difficult conversations with family, friends or medical professionals.
I simplify the complex
As a board-certified palliative social worker with experience in hospital-based bioethics consultations, I have expertise in facilitating advance care planning conversations and completing advance care planning documentation.
I get to know you, your values, circumstances and concerns
You tell me about your people — the important, trusted people in your life
Gain an understanding of the relevant documents and the ways they are used
We discuss your wishes and explore potential consequences for you and loved ones
When ready, we can document your wishes in an Advance Directive.
Yes. Really. The time is now to plan.
It is never too soon to think about your wishes and having discussions with your most important people. Whether you are young and healthy, advanced in age or living with illness, the time is now.
Early and ongoing planning gives you a voice in medical decision making for your future self, should there come a time when you are unable to express your own wishes.
Beginning now can spare those closest to you the difficulty of making decisions on your behalf without your input. It can be the ultimate gift you give.
